GRAD
BC grade 12 students - If you are graduating from a BC secondary school this year, you can have your final secondary school grades sent directly to the university of your choice.
This option is only available in online format. To access the "Post-Secondary Selection Form" to tell the Ministry of Education that you'd like to have your grades sent, log in at www.bced.gov.bc.ca/exams/tsw/tsw/student. You should see "Welcome to the Student Secure Web". Click ENTER. You will be prompted to log in or create a new account if you don't already have one. Follow the instructions carefully to submit your choice of Post-Secondary Institutions.
CSS will provide information for the Graduates of 2012 as available.

Grad Transition Planning Booklet Information
1) It is expected that the white pages are read and completed by Grade 12’s – these pages are to be used as a guide/resource for Grade 12’s (they will not be collected for marks)
2) Yellow sections to be completed and handed in to the office for GT Credit
Module 1 Personal Health
DPA – Daily Physical log is completed by those students who are not taking PE in either semester or completed for one semester and handed in at the end of the semester (it is not expected to be completed for the GT deadline)
Module 2 Self Evaluation of Work/Volunteer Experience
Page 65 – a signature must be obtained for verification of 30 hours of either paid or volunteer experience from either an employer, teacher or parent
Page 66 – must be filled out by the student as a self-evaluation of their experience
Module 3 “Your Plan for Your Future” one option must be completed
Option 1
a) Attend a “Life After High School” workshop
AND
b) Attend a follow-up appointment with Mrs. Hill
OR
Option 2 “Personal Based GT Project”
Complete Exit Project Part 1 and 2 on pages 87-88
